Description: A 75 year old patient was referred with history of gradual deterioration of vision in both eyes after 3 months of her routine cataract surgery with hydrophobic acrylic IOLs. The referring surgeon identified ‘opaque IOLs’ and referred for further management. She was noted to have a best corrected visual acuity of 20/20 one month postoperatively but this had deteriorated to 20/120 by 3 months. Detailed examination revealed opaque anterior surface of the hydrophobic acrylic IOLs in both eyes.
